If you cannot collect your transcript(s) or any required statement, you can authorise someone else to collect the documents at Student Services.

Below you will find the link to the authorisation and permission collecting form. Print this form, fill in all the information and give it to the delegate who will collect your documents at the Student Services Desk.

Please note

The person who collects your documents (your delegate) has to legitimise themselves at the Student Services desk. They also have to hand in this form with a copy of their ID/passport at the Student Services desk.
