Mandatory documents:
- motivation letter*
- a complete and detailed study progress overview (SVO/SPO) from Osiris
- a study plan (drawn up in cooperation with the study advisor)
- if applicable: TOP Sport or Art student: decision of CvB (CPO)
- if applicable: declaration of personal circumstances (For EB GS, IS and MS from study advisor; EB BS via EAO**)
- if applicable: board year: the appointment letter from FOBOS or student association, including the appointment period and proof of the financial compensation
* mandatory data in your motivation letter:
- student name and last name
- student number
- programme you are enrolled in
- articles applicable to your request
- clear substantiation for your request
- (a list of) the study unit(s)/module name(s) and code(s) you would like extension for
- proposed period of extension
- if applicable: result(s) of specific (sub)test(s) (incl. date(s) and date of the next regular planned exam/re-sit
- name examiner

Please note:
- The request has to be submitted at the latest 6 weeks*** before the start of the study unit in order to receive a decision before the start of the study unit. The examination board may postpone the handling of a late requests until after the end of the (part of the) study unit concerned.
- Pending the decision you are strongly advised to participate in the study unit and to take part in the (next) regular scheduled exam/test.
*** The 6-week period is set because of the response time for the examination board to prevent unnecessary disturbance of study units such as the falling apart of working groups during a study unit (in several parts of the study unit ) due to the withdrawal of a student.
