ISALA@HOME: A NEW REALITY. LESSONS FROM THE ISALA CONNECTED CARE PROGRAM

Abstract
Isala started it’s Connected Care Center (CCC) in 2019. This new department of the hospital was established to stimulate the development and growth of hospital care at home.
The phrase "connected care" refers to the connection with the hospital that the patient has for this care. Forms of connected care include telemonitoring, online consultation and control interviews, also in combination with home visits by a specialized nurse. Connected care often uses special measuring equipment, also known as point of care applications, which is connected to the hospital via the internet and which can be read at Isala.
The Connected Care center supports health care professionals and hospital departments in the redesign of the care delivery chain from hospital to home. During the presentation successes, failures and dilemmas will be shared with the audience.
