The information you provide here about your personal circumstances will be handled by Student Affairs, Coaching & Counselling (SACC). Your data will be registered in the information system of the UT and will be treated confidentially.
In order to be able to advise and counsel you properly, the information you provide will only be shared with the study advisor of your programme with your permission. The study adviser will also treat this information as confidential. In this form, we ask you to give us permission to share your information with the study adviser of your programme.
