In order to qualify for support facilities, a medical certificate is required. This certificate justifies the application for facilities and schemes on the grounds of personal circumstances. If you think you need support facilities, bring a medical statement stating your diagnosis with you when applying for the facility. Furthermore, students must supply a translation of their medical (or dyslexia/dyscalculia) certificate stating the diagnoses in English.
- Illness, disability/impairment, psychological problems: a medical certificate from a GP or BIG-registered psychologist.
- Dyslexia diagnosis: issued by a qualified mental health psychologist or remedial educationalist (BIG-registered), and must also meet the criteria of the Dyslexia Foundation of the Netherlands (SDN).
- Dyscalculia diagnosis: issued by a qualified mental health psychologist or remedial educationalist (BIG-registered).
If necessary, you can use the standard model for a medical certificate.
