DAILY MONITORING AND TAILORED COACHING TO IMPROVE PHYSICAL ACTIVITY, DIET AND GLYCEMIC CONTROL IN PEOPLE WITH TYPE 2 DIABETES

Type 2 Diabetes (T2D) is one of the most common chronic diseases worldwide with major impact on the health and well-being of individuals. Obesity, an unhealthy diet and a sedentary lifestyle are important risk factors for the development of T2D, impairment of glycemic regulation, and the development of diabetes-related complications. Lifestyle modification therefore is a successful strategy to manage or even reverse T2D. A considerable proportion of people with T2D who improved their lifestyle obtain better glycemic control and a reduced need for medication. Due to the complexity of the behavioral change process, there is a demand for intensive and personal lifestyle guidance.

The DIAMETER project is a collaboration between the University of Twente and Ziekenhuisgroep Twente (ZGT). The DIAMETER is a mobile app that aims to improve lifestyle habits and glycemic control in people with T2D with the result that they experience better well-being and develop fewer diabetes-related complications in the long term. The app offers monitoring of physical activity (i.e., through an activity tracker and self-reported activities), nutrition (i.e., through a digital food diary built into the app), and glucose levels (i.e., through a continuous glucose monitoring sensor). Furthermore, people with T2D receive tailored coaching advice to improve their lifestyle by means of goal-setting, motivational messages, feedback, and supportive exercises. The DIAMETER is used as a supplement to regular care in order to provide healthcare professionals with objective insight into their patients' lifestyle and glycemic regulation to offer more personalized treatment.
