Host Evaluation Form

Dear Host Supervisor,

Thank you very much for supervising our student.

We truly appreciate this internship opportunity at your organisation, and we hope that the internship assignment and its outcome fulfilled your organisation’s needs.

You are kindly requested to complete the Host Evaluation form, preferably as soon as possible after the internship has ended, the internship report has been submitted, and the student has presented the results. You can access and submit the Host Evaluation form through our online platform. Please find the details below.

1. Go to the ITC Student Assessment (ISA) environment: http://isa.itc.utwente.nl/
2. Click on ‘Login with access code’ and add your email address and the received access code.
   - The access code is sent to you by email. Please also check your SPAM folder.
   - In case you need a new code, you can contact us via: internship-itc@utwente.nl
   
    
   
3. You should now have access to the following:
   - The internship details and submitted reports from your student/intern
   - The host evaluation form to evaluate your student/intern:
   - Click on each rubric (in blue)
   - Assess each rubric by selecting a mark
   - Click on submit at the bottom of the page to submit your evaluation.
   
   
4. The ITC Internship Coordinator, ITC supervisor and student/intern will automatically receive a copy of the submitted Host Evaluation form.
