Real-world data monitoring and AI for personalized digital care for people with chronic conditions

RE-SAMPLE is a European Horizon project on the use of real-world data (RWD) and AI to monitor and predict flare-ups of chronic obstructive pulmonary disease (COPD) and complex chronic conditions (CCCs). It aims to develop tailor-made care for shared decision-making and self-management.
In this webinar, the RE-SAMPLE team will present the project’s key results about data-driven healthcare tools. This includes insight in new parameters from RWD for monitoring COPD and CCCs, AI-prediction models for flare-ups of COPD and CCCs, a privacy-preserving platform, and self-management strategies.
The project will present its open science strategy and provide guidelines on how personalised digital health tools can be integrated in pan-European chronic healthcare settings.
