Human Error is an advanced course in which human error is studied from a systems perspective.

This course deals with human error from multiple perspectives. Both cognitive, team as well as organizational aspects will be discussed. In the lectures, we will deal with both methodological (e.g., types of accident investigations; perspectives on human error) and thematic aspects (e.g., trust in automation; naturalistic decision making; cooperation in teams). Emphasis will be placed on translating fundamental knowledge to well-known accidents in civil aviation (Tenerife, Ueberlingen), space (Challenger), process industry (Three Mile Island), military systems (USS Vincennes), the medical domain, and the maritime industry (Exxon Valdez).

Apart from specific knowledge, the skills in reading and writing scientific articles will be enhanced by giving a presentation on a scientific article and/or accident investigation report that has been studied and writing a weekly essay. A final exam in which a novel accident investigation report has to be studied will conclude the course.
